Supportive Housing

At Community Builders, we follow the innovative Whole Life program model, centered around empowering residents for independent living success. Our person-centered approach involves comprehensive case planning to address specific needs and preferences. Residents are encouraged to identify support gaps, and our staff actively assists in fulfilling those requirements, be it primary care, substance use treatment, or income needs. We advocate on behalf of our residents, liaising with third-party providers and their self-identified circle of care.

To ensure housing retention and minimize eviction rates, participation in the Whole Life program is mandatory for all our supportive housing residents. We provide a wide range of services, including home support, a nutritious meal program, laundry facilities, maintenance assistance, and pest control. With the end goal of transitioning into permanent housing, our team works closely with each resident on an individualized basis to succeed. At Community Builders, we are committed to creating a safe and nurturing environment, fostering a sense of community and helping our residents thrive on their journey to a fulfilling, independent life.
